Decides whether a model runs, runs after the user's approval, or never runs.
Parameters
- model: string
  The concrete model the turn would run.
- defaultModel: string | undefined
  The backend's default model, the price reference; undefined when none is known.
- prices: StoredPrices | undefined
  The price list, or undefined when none could be read.
- settings: GovernanceSettings
  The user's governance settings.
- Optionalrun: EffortRun
  The reasoning effort the turn passes separately and how the CLI applies it.
